End-stage renal failure patients on dialysis show impaired thyroid function. Continuous ambulatory peritoneal dialysis (CAPD) did not benefit thyroid function compared to haemodialysis (HD).

    Area of Science:

    • Nephrology
    • Endocrinology
    • Internal Medicine

    Background:

    • End-stage renal failure (ESRD) significantly impacts various physiological systems.
    • Thyroid hormone metabolism is often altered in patients with chronic kidney disease.

    Purpose of the Study:

    • To evaluate and compare thyroid function in patients with ESRD undergoing maintenance haemodialysis (HD) versus continuous ambulatory peritoneal dialysis (CAPD).
    • To assess the impact of different dialysis modalities on thyroid hormone levels and TSH response.

    Main Methods:

    • Study included 16 ESRD patients (8 HD, 8 CAPD) and 8 healthy controls.
    • Measured serum total thyroxine (T4), triiodothyronine (T3), free thyroxine (FT4), basal thyroid-stimulating hormone (TSH), and TSH response to thyrotrophin-releasing hormone.

    Related Experiment Videos

    Main Results:

    • Both HD and CAPD groups exhibited low serum T4 and T3 levels compared to controls.
    • T3 levels were significantly lower in the CAPD group than the HD group.
    • Serum FT4 was low in CAPD patients, and TSH responses to TRH were blunted in both dialysis groups.

    Conclusions:

    • Neither HD nor CAPD offers a beneficial effect on thyroid function in ESRD patients.
    • Despite potential advantages in other parameters, CAPD does not improve thyroid hormone profiles over HD.
