    Background:

    • Ankylosing spondylitis (AS) commonly causes back pain and spinal fractures.
    • Neurological deficits are rare in AS patients, typically not involving nerve roots.

    Observation:

    • A patient with AS presented with spinal claudication and urinary sphincter dysfunction.
    • Imaging revealed severe lumbosacral spinal stenosis not clearly seen on myelography.

    Findings:

    • Surgical exploration identified extensive posterior soft tissue ossification as the cause of stenosis.
    • This ossification led to significant compression of neural structures.

    Implications:

    • Posterior soft tissue ossification is a previously unrecognized complication of AS.
