    Summary

Metroplasty significantly improves fetal survival rates in women with double uterus, increasing success from 0% to over 92%. Different surgical techniques offer varying benefits for specific uterine malformations.

    Area of Science:

    • Reproductive Medicine
    • Gynecologic Surgery
    • Maternal-Fetal Medicine

    Background:

    • Double uterus (uterus didelphys) is a congenital uterine malformation.
    • This condition is often associated with infertility and poor pregnancy outcomes, including high rates of miscarriage and preterm birth.

    Purpose of the Study:

    • To evaluate the effectiveness of various metroplasty techniques in improving fetal survival rates for patients with double uterus.
    • To compare the outcomes and applicability of different surgical methods for correcting uterine malformations.

    Main Methods:

    • A retrospective analysis of 32 patients who underwent metroplasty for double uterus.
    • Surgical procedures included Strassmann's operation (16 cases), Jones & Jones's modified method (12 cases), and Tompkins's operation (4 cases).
    • Diagnosis was confirmed by hysterosalpingography (HSG) and direct observation.

    Main Results:

    • Preoperative fetal survival rate was 0%; postoperative survival reached 92.6%.
    • Strassmann's operation is suited for bicornuate uteri, while Jones & Jones's method is effective for partial septate uteri.
    • Postoperative fundal deformation was more common with Strassmann's method than the modified Jones & Jones technique.

    Conclusions:

    • Metroplasty is a viable option for women with uterine malformations experiencing primary infertility after other causes are excluded.
    • Accurate diagnosis combining HSG and direct observation is crucial for selecting the appropriate surgical technique.
    • Careful surgical technique to prevent adhesions and appropriate postoperative management, including contraception and delivery method, are essential for successful outcomes.
