    Background:

    • Intracapsular cataract extraction (ICCE) is a surgical procedure to remove the lens.
    • Complications following cataract surgery can impact visual outcomes.
    • Anterior chamber collapse and hypotony are serious postoperative concerns.

    Observation:

    • A 60-year-old woman developed unexplained anterior chamber collapse, choroidal and ciliary body detachments, and ocular hypotony two weeks after an uneventful ICCE.
    • Initial attempts to maintain anterior chamber depth with air were unsuccessful.

    Findings:

    • Surgical drainage of suprachoroidal fluid was performed.
    • Sodium hyaluronate was injected into the anterior chamber.
    • This intervention facilitated spontaneous recovery of aqueous production.
    • The anterior segment was protected from further damage.

    Implications:

    • Sodium hyaluronate is a viable treatment option for managing hypotony and anterior chamber collapse after cataract surgery.
    • Prompt intervention can prevent long-term complications and preserve vision.
    • This case highlights the importance of recognizing and managing delayed postoperative hypotony.
