The thyroglossal cyst

R H Allard

    Head & Neck Surgery

    Summary

Thyroglossal cysts are common nonodontogenic neck cysts. Differentiating benign cysts from malignancy is clinically impossible, necessitating a thorough review of all aspects.

    Area of Science:

    • Head and Neck Surgery
    • Oral and Maxillofacial Pathology
    • Developmental Biology

    Background:

    • Thyroglossal duct cysts represent the most frequent nonodontogenic cysts encountered in the neck.
    • While typically originating in the neck, these cysts can less commonly present in the lingual or submental regions.
    • Malignant transformations within thyroglossal cysts have been documented, posing diagnostic challenges.

    Purpose of the Study:

    • To comprehensively review the existing literature on thyroglossal duct cysts.
    • To elucidate the embryological origins, epidemiological patterns, and etiological factors associated with these cysts.
    • To detail the clinical presentation, diagnostic imaging, histological features, and therapeutic strategies for both benign and malignant thyroglossal duct cysts.

    Main Methods:

    • Systematic literature review of thyroglossal duct cysts and their malignant counterparts.
    • Analysis of studies covering embryology, epidemiology, etiology, symptomatology, radiology, and histology.
    • Synthesis of current treatment modalities and outcomes.

    Main Results:

    • Thyroglossal cysts are the most prevalent nonodontogenic cysts in the neck.
    • Differential diagnosis between benign cysts and malignancy is not clinically feasible.
    • A comprehensive understanding of the cyst's lifecycle, from development to treatment, is crucial.

    Conclusions:

    • Thyroglossal duct cysts are common developmental anomalies with potential for malignant change.
    • Clinical presentation does not reliably distinguish between benign and malignant forms.
    • A thorough understanding of embryology, pathology, and clinical presentation is essential for effective management.
