    Summary

    The enterohepatic circulation (EHC) of bile acids shows significant, irregular variations linked to intestinal peristalsis. This finding may explain fluctuations in blood bile acid levels in patients with liver disease.

    Background:

    • The enterohepatic circulation (EHC) of bile acids is crucial for their reabsorption and reuse.
    • Previous studies have not fully elucidated the causes of variability in bile acid EHC.

    Purpose of the Study:

    • To investigate the factors contributing to variations in bile acid enterohepatic circulation in a canine model.
    • To determine the relationship between intestinal peristalsis and the rate of bile acid EHC.

    Main Methods:

    • Studies were conducted on fasting dogs with a portacaval shunt in a steady state.
    • Bile acid EHC was measured at 15-minute and hourly intervals.
    • The effects of drugs influencing intestinal motility (bethanechol chloride, diphenoxylate HCl) were assessed.

    Main Results:

    • The rate of bile acid EHC was proportional to systemic blood bile acid concentration.
    • Significant, irregular variations (20-100%) in bile acid EHC were observed.
    • These variations persisted after cholecystectomy and sphincterotomy.
    • Enhanced intestinal peristalsis increased EHC irregularities, while slowed peristalsis suppressed them.

    Conclusions:

    • Irregular patterns of intestinal peristalsis are a primary cause of variability in bile acid EHC.
    • This peristaltic influence on EHC may account for observed variations in blood bile acid concentrations in liver disease patients.
