    Summary

    Selective IgA deficiency patients were studied for low molecular weight (LMW) IgM. LMW IgM was found in 32% of patients, and its presence correlated with clinical disease and immune complex formation.

    Background:

    • Selective IgA deficiency is a primary immunodeficiency.
    • The role of specific immunoglobulin abnormalities in disease manifestation is under investigation.

    Purpose of the Study:

    • To investigate the prevalence of low molecular weight (LMW) serum IgM in individuals with selective IgA deficiency.
    • To explore the association between LMW IgM and clinical disease or immune complex formation in this population.

    Main Methods:

    • Study population: 39 individuals with selective IgA deficiency.
    • Detection of LMW IgM using sensitive gel filtration in 28 subjects.
    • Immune complex detection via C1q-dependent radioimmunoassay in 7 subjects.

    Main Results:

    • LMW IgM was detected in 32% (9/28) of individuals with selective IgA deficiency.
    • In positive cases, LMW IgM comprised up to 17% of total serum IgM.
    • Eight of nine individuals with LMW IgM had clinical disease, compared to the general IgA-deficient cohort. Serum immune complexes were found in 5/7 subjects with LMW IgM, with 4 experiencing immune complex-associated disorders.

    Conclusions:

    • The presence of LMW IgM in selective IgA deficiency may be linked to disease development.
    • LMW IgM associated with IgA deficiency might contribute to pathogenic immune complex formation, leading to clinical manifestations like polyarthritis and glomerulonephritis.
