    Background:

    • Epibulbar tumors are growths on the surface of the eye.
    • Neurofibromas are benign tumors arising from nerve sheath cells.
    • Isolated episcleral neurofibromas are exceptionally rare presentations.

    Observation:

    • A 22-year-old woman presented with a long-standing epibulbar tumor.
    • Clinical examination revealed a lobulated, yellowish-white mass on the inferior bulbar conjunctiva.
    • The tumor was fixed to the sclera with smaller satellite nodules.

    Findings:

    • Histopathologic evaluation confirmed the diagnosis of an isolated episcleral neurofibroma.
    • The tumor was excised with clear margins, showing no conjunctival attachment.
    • No other systemic neurofibromatosis stigmata were reported.

    Implications:

    • Isolated episcleral neurofibromas must be considered in the differential diagnosis of epibulbar masses.
    • Conservative management is often appropriate for these rare tumors.
    • Further research into the etiology and optimal treatment of episcleral neurofibromas is warranted.
