    Vestibular neuronitis diagnosis may be misleading. Many patients diagnosed with this peripheral nerve disorder actually have an underlying central nervous system (CNS) disorder, requiring further investigation.

    Background:

    • Vestibular neuronitis is a common diagnosis for vertigo.
    • Routine examinations may not identify the underlying cause of vertigo.

    Purpose of the Study:

    • To investigate the central nervous system (CNS) involvement in patients diagnosed with vestibular neuronitis.
    • To differentiate between peripheral vestibular nerve disorders and intrinsic CNS disorders causing vertigo.

    Main Methods:

    • A comprehensive test battery including neuro-otological, neuro-ophthalmological, and neurological examinations.
    • Electro-oculography and cerebrospinal fluid analysis were performed.

    Main Results:

    • Routine examinations initially failed to detect extratubal involvement.
    • Neurological evaluation and cerebrospinal fluid analysis revealed CNS disorders in 6 cases.
    • Electro-oculography indicated central vertigo causes in 12 cases, with 13 patients showing signs of intrinsic CNS disorders.

    Conclusions:

    • Many patients diagnosed with vestibular neuronitis may have an underlying intrinsic CNS disorder.
    • Comprehensive neuro-otological and neurological assessments are crucial for accurate vertigo diagnosis.
    • Distinguishing between peripheral and central causes of vertigo is essential for appropriate patient management.
