Immunoglobulin D (IgD) myeloma, a rare and aggressive cancer, can manifest as skin nodules. This aggressive hematologic malignancy often involves soft tissues and progresses rapidly.

    Area of Science:

    • Hematology
    • Oncology
    • Dermatology

    Background:

    • Immunoglobulin D (IgD) myeloma is a rare subtype of plasma cell neoplasm.
    • It is characterized by its occurrence at a younger age and aggressive clinical course.

    Observation:

    • A 53-year-old man presented with multiple firm, nontender, violaceous cutaneous and subcutaneous nodules on the abdomen and thighs.
    • Biopsy revealed large atypical undifferentiated cells in the dermis, consistent with IgD myeloma.
    • Autopsy demonstrated widespread dissemination, including osteolytic lesions and extensive tumor involvement of soft tissues and organs.

    Findings:

    • IgD myeloma exhibits a high propensity for soft tissue infiltration, presenting as dermal and subcutaneous nodules.
    • The disease is characterized by dedifferentiation and rapid progression, leading to widespread metastasis.

    Related Experiment Videos

  • Malignant lymphoid cells replaced bone marrow and infiltrated various organs, including kidneys and intestines.
  • Implications:

    • This case highlights the significant extranodal involvement characteristic of IgD myeloma.
    • Early recognition of cutaneous manifestations may be crucial for timely diagnosis and management of this aggressive hematologic malignancy.
    • Further research into the unique biology and treatment strategies for IgD myeloma is warranted.
