Localized scleroderma and idiopathic thrombocytopenia

S H Neucks, T L Moore, J R Lichtenstein

    The Journal of Rheumatology
    |September 1, 1980

    Summary

Localized scleroderma can be associated with thrombocytopenic purpura, a serious condition affecting platelets. Prednisone treatment resolved the purpura in two patients, suggesting a potential immune link.

    Area of Science:

    • Dermatology
    • Immunology
    • Hematology

    Background:

    • Localized scleroderma is a connective tissue disease primarily affecting the skin.
    • Thrombocytopenic purpura is characterized by low platelet counts, leading to bleeding risks.

    Observation:

    • Two patients with localized scleroderma presented with secondary thrombocytopenic purpura.
    • Both patients exhibited positive antinuclear antibody (ANA) tests.
    • One patient also had positive immune complex assays.

    Findings:

    • Thrombocytopenia resolved in both patients following prednisone therapy.
    • Platelet counts remained stable during extended follow-up periods without steroid use.
    • No alternative non-immune causes for thrombocytopenia were identified.

    Related Experiment Videos

    Implications:

    • This case series suggests a potential autoimmune association between localized scleroderma and thrombocytopenic purpura.
    • The findings highlight a possible new, serious complication of localized scleroderma.
    • Early recognition and management of thrombocytopenia may be crucial in affected patients.
