    Background:

    • Luteal insufficiency is a potential cause of female infertility.
    • This condition is observed in women with hyperprolactinemia and galactorrhea.
    • Normoprolactinemic women undergoing clomiphene treatment may also present with galactorrhea and luteal insufficiency.

    Observation:

    • Fifteen cases of female infertility associated with luteal insufficiency were analyzed.
    • Elevated prolactin levels were frequently noted in these cases.
    • Galactorrhea was a common symptom in both hyperprolactinemic and clomiphene-treated normoprolactinemic women.

    Findings:

    • Bromocryptine effectively reduces prolactin levels.
    • Treatment with bromocryptine leads to a prolonged luteal phase.
    • Progesterone secretion is increased following bromocryptine treatment.
    • An optimal prolactin concentration appears necessary for adequate luteal function.

    Implications:

    • Measurement of prolactin and progesterone levels during the luteal phase is recommended for diagnosing luteal insufficiency.
    • Basal body temperature charting can monitor the hyperthermic phase before and after treatment.
    • Understanding the prolactin-progesterone relationship is crucial for managing infertility related to luteal defects.
