    Background:

    • Chronic otitis media is a prevalent condition.
    • The role of immune complexes and complement factors in otitis media pathogenesis is not fully understood.

    Purpose of the Study:

    • To investigate the presence and quantity of immune complexes in middle ear effusion (MEE) and serum.
    • To assess serum levels of complement (C) factors in patients with chronic otitis media.
    • To explore the relationship between complement system function and disease activity.

    Main Methods:

    • Analysis of immune complexes in MEE and serum samples.
    • Quantification of serum complement factors, including Clq and properdin.
    • Assessment of complement system activation pathways.

    Main Results:

    • Immune complexes were detected in 85% of serous MEE and 28% of sera.
    • Depressed Clq values and abnormal complexes indicated disturbed complement function.
    • Classical pathway complement activation was observed in 23% of patients.
    • Decreased properdin levels were noted.
    • Complement system abnormalities tended to normalize as otitis subsided.

    Conclusions:

    • Immune complexes play a significant role in the pathophysiology of chronic otitis media.
