Altered bile in diabetic diarrhoea

A M Molloy, G H Tomkin

    British Medical Journal
    |November 25, 1978
    PubMed
    Summary

Diabetic patients with neuropathy or diarrhea show significant changes in bile salt composition and excretion. These bile abnormalities may contribute to the development of diabetic diarrhea.

    Area of Science:

    • Gastroenterology
    • Endocrinology
    • Metabolic Disorders

    Background:

    • Diabetes mellitus is associated with various complications, including gastrointestinal issues.
    • Alterations in bile acid metabolism can impact digestive function.
    • Diabetic neuropathy and diarrhea are common, but their relationship with bile salt pools is not fully understood.

    Purpose of the Study:

    • To compare bile salt pool size and composition in diabetics with neuropathy, diabetic diarrhea, and uncomplicated diabetes.
    • To investigate potential links between bile salt abnormalities and diabetic diarrhea.

    Main Methods:

    • Analysis of bile salt pool size and composition.
    • Measurement of fecal bile salt excretion.
    • Comparison between three groups: diabetics with neuropathy, diabetics with diarrhea, and uncomplicated diabetics.

    Main Results:

    • Diabetics with neuropathy had increased dihydroxy bile salts, larger bile salt pools, and higher fecal bile excretion compared to controls.
    • Diabetics with diarrhea exhibited increased dihydroxy bile salts, a higher glycine to taurine ratio, smaller bile salt pools, and increased excretion of 14C-tracer and total bile salts.

    Conclusions:

    • Significant alterations in bile salt metabolism occur in diabetics with neuropathy or diarrhea.
    • These bile acid abnormalities may play a role in the pathogenesis of diabetic diarrhea.
