    Herpes simplex virus type 2 (HSV-2) can cause aseptic meningitis in homosexual men with anal herpes. This sexually transmitted infection requires consideration in meningitis diagnoses.

    Background:

    • Healthcare providers should consider HSV-2 in the differential diagnosis of aseptic meningitis, particularly in patients with a history of or risk factors for sexually transmitted infections.
    • This case expands the known clinical spectrum of HSV-2 neurological complications.
    • Further research may be warranted to understand the incidence and mechanisms of HSV-2-associated meningitis from anal infections.
