    Summary

    Diurnal variations in cobalamin-binding proteins like transcobalamins (TC-II and R-binders) showed minor fluctuations. However, significant nighttime decreases in P-cobalamin and P-R-binders necessitate standardized collection times for clinical monitoring.

    Background:

    • Cobalamin (Vitamin B12) transport in plasma involves binding proteins, primarily transcobalamins (TC-II and R-binders).
    • Understanding diurnal variations in these binding proteins is crucial for accurate clinical interpretation.

    Purpose of the Study:

    • To investigate the diurnal variations of cobalamin-binding plasma proteins in healthy individuals.
    • To assess the impact of these variations on the reliability of using these proteins as biomarkers.

    Main Methods:

    • 10 healthy volunteers participated in the study.
    • Unsaturated cobalamin-binding capacities of transcobalamins (TC-II and R-binders), P-cobalamin, and P-protein were measured every 3 hours over a 24-hour period.
    • Measurements were analyzed for diurnal patterns and corrected for total plasma protein variations.

    Main Results:

    • Overall diurnal variations were within +/- 20%.
    • Significant decrements in cobalamin-binding proteins were observed during nighttime hours.
    • After adjusting for total plasma protein, P-cobalamin and P-R-binders still showed significant decreases (12% and 8% respectively) during the night.

    • Diurnal variations in cobalamin-binding plasma proteins, particularly P-cobalamin and P-R-binders, are significant, especially during nighttime.
    • Standardized collection times and patient posture are essential when utilizing these proteins for monitoring malignant, hematological, and autoimmune disorders.
