    Summary

    Acute subdural hematoma can arise from bleeding aneurysms or arteriovenous malformations. Computed tomography alone may miss these critical diagnoses in head-injured patients.

    Background:

    • Acute subdural hematoma (ASH) is a critical neurosurgical emergency.
    • Vascular malformations, such as aneurysms and arteriovenous malformations (AVMs), are potential causes of intracranial hemorrhage.

    Observation:

    • This study presents three cases of ASH associated with ruptured aneurysms or AVMs.
    • The arterial sources contributing to subdural hematoma formation were reviewed.
    • The intricate relationship between vascular malformations and ASH in head-injured patients is explored.

    Findings:

    • Ruptured aneurysms and AVMs represent significant, though less common, causes of acute subdural hematoma.
    • Computed tomography (CT) may not be sufficient as a sole diagnostic tool for evaluating head-injured or unconscious patients.

    • Enhanced diagnostic vigilance is required for patients with head trauma, particularly when initial CT scans are inconclusive.
    • Considering vascular malformations as a potential etiology for ASH can improve diagnostic accuracy and patient outcomes.
    • Multimodal diagnostic approaches may be necessary for comprehensive evaluation in complex head injury cases.
