    Hairy-cell leukemia (HCL) patients face a high risk of pulmonary infections. Early consideration of granulomatous infections, especially mycobacterioses, is crucial for effective HCL treatment and patient survival.

    Background:

    • Hairy-cell leukemia (HCL) is a rare hematologic malignancy.
    • HCL patients exhibit increased susceptibility to pulmonary infections, including opportunistic pathogens.
    • Granulomatous lung infections, particularly mycobacterioses, are associated with HCL.

    Purpose of the Study:

    • To investigate the incidence and outcomes of pulmonary infections in Hairy-cell leukemia patients.
    • To highlight the importance of considering granulomatous infections in the differential diagnosis of pulmonary disease in HCL.

    Main Methods:

    • Retrospective analysis of 33 Hairy-cell leukemia patients.
    • Identification and classification of pulmonary infections (fungal, mycobacterial).
    • Evaluation of treatment responses and patient survival.

    Main Results:

    • Nine out of 33 HCL patients (27%) presented with fungal or mycobacterial infections.
    • Five patients had infections caused by atypical mycobacterial species.
    • Appropriate therapy led to survival in 5 out of 6 patients with mycobacterioses and 1 out of 3 with fungal pneumonia.

    Conclusions:

    • Pulmonary infections are a significant complication in Hairy-cell leukemia.
    • Mycobacterioses and other granulomatous infections should be strongly considered in HCL patients with pulmonary parenchymal disease.
    • Timely diagnosis and treatment of these infections improve patient outcomes.
