    Summary

    Postoperative irradiation improves survival rates for malignant thymoma patients. This treatment is recommended for all malignant thymoma cases, regardless of surgical intervention extent, to reduce recurrence and mortality.

    Background:

    • Thymoma is a rare tumor of the thymus gland.
    • Treatment strategies for thymoma vary based on malignancy and resectability.
    • Optimal adjuvant therapy for malignant thymoma remains debated.

    Purpose of the Study:

    • To evaluate the impact of postoperative radiotherapy on survival outcomes in patients with thymoma.
    • To compare survival rates between irradiated and non-irradiated malignant thymoma patients.

    Main Methods:

    • Retrospective analysis of 40 patients diagnosed with thymoma.
    • Patients were categorized into benign and malignant thymoma groups.
    • Treatment modalities included surgical resection (total or partial) and radiotherapy.

    Main Results:

    • All 18 patients with benign thymoma treated with resection alone showed no recurrence or disease-related death.
    • For malignant thymoma, 15 of 20 patients received radiotherapy post-surgery.
    • Irradiated malignant thymoma patients had a 5-year survival of 45.6% and 10-year survival of 34.4%, compared to only one long-term survivor among 6 non-irradiated patients.

    Conclusions:

    • Postoperative radiotherapy significantly improves survival for patients with malignant thymoma.
    • Adjuvant irradiation should be considered for all malignant thymoma cases, irrespective of initial surgical success.
    • Surgical resection alone is highly effective for benign thymoma, with excellent long-term outcomes.
