Hemodialysis and thyroid functions in children

    Summary

    Children on maintenance hemodialysis often have low thyroid hormone levels (T3, T4) and thyroid-stimulating hormone (TSH). Hemodialysis itself did not significantly alter these thyroid function tests in the study group.

    Background:

    • Chronic kidney disease (CKD) and its treatment, such as hemodialysis, can impact endocrine function.
    • Thyroid dysfunction is a potential complication in children with end-stage renal disease (ESRD).

    Purpose of the Study:

    • To evaluate thyroid function in pediatric patients undergoing maintenance hemodialysis.
    • To assess the immediate impact of hemodialysis on thyroid hormone levels.

    Main Methods:

    • Serum concentrations of triiodothyronine (T3), thyroxine (T4), and thyroid-stimulating hormone (TSH) were measured in 16 children on maintenance hemodialysis.
    • Measurements were taken before and after hemodialysis sessions.

    Main Results:

    • Children on maintenance hemodialysis exhibited lower than age-normal serum concentrations of T3, T4, and TSH.
    • No significant changes in T3, T4, or TSH levels were observed immediately before and after hemodialysis.

    Conclusions:

    • Pediatric patients on maintenance hemodialysis may present with subnormal thyroid hormone levels and TSH.
    • The study did not investigate the underlying causes of these thyroid function abnormalities, such as chronic uremia's effect on TSH secretion.
