Ulcerative colitis complicated by autoimmune hemolytic anemia

Background:

Ulcerative colitis (UC) is a chronic inflammatory bowel disease.
Autoimmune hemolytic anemia (AIHA) is a condition where the immune system attacks red blood cells.
The co-occurrence of severe AIHA and active UC is rare, with limited management guidelines.

Observation:

A patient presented with severe AIHA and active UC.
Aggressive medical therapies provided only partial control of both UC and AIHA.
Surgical intervention was considered due to the limited response to medical management.

Findings:

Panproctocolectomy (surgical removal of the entire colon and rectum) and splenectomy (surgical removal of the spleen) were performed.

Both severe AIHA and active UC demonstrated rapid remission following the surgical procedures.

Autologous red blood cell transfusion during surgery was safely administered.

Implications:

Surgical management, specifically panproctocolectomy and splenectomy, may be an effective treatment for the rare concurrence of severe AIHA and active UC.
This case highlights the potential benefits of surgical intervention when medical management is insufficient.
Further research is warranted to establish definitive treatment protocols for this rare clinical presentation.
