    Summary

    Rheumatoid arthritis can impair lung gas transfer, primarily due to reduced pulmonary capillary volume (Vc), especially in smokers and those with nodules. Corticosteroid treatment may mitigate this effect.

    Background:

    • Rheumatoid disease is associated with abnormal lung gas transfer in some patients.
    • Pulmonary involvement is a known complication of rheumatoid arthritis.

    Purpose of the Study:

    • To investigate the components of gas transfer, specifically membrane component (Dm) and pulmonary capillary volume (Vc), in rheumatoid arthritis patients.
    • To identify factors influencing gas transfer abnormalities in rheumatoid arthritis.

    Main Methods:

    • Measured Dm and Vc in 48 rheumatoid arthritis patients and 48 matched healthy volunteers.
    • Compared gas transfer parameters between patient and control groups, and analyzed effects of smoking, medication, and clinical features.

    Main Results:

    • No significant difference in Dm between rheumatoid arthritis patients and controls.
    • Significantly lower Vc observed in male rheumatoid arthritis smokers compared to male control smokers.
    • Rheumatoid arthritis female smokers had significantly lower Vc than non-smokers.
    • Vc was significantly lower in patients receiving corticosteroids and those with nodules.
    • Persistently low transfer factor correlated with significantly lower Vc.

    Conclusions:

    • The observed abnormality in rheumatoid arthritis gas transfer is primarily attributed to a reduction in pulmonary capillary volume (Vc).
    • Pulmonary blood vessel involvement may occur in rheumatoid arthritis patients with nodules and could be suppressed by corticosteroid therapy.
