    Background:

    • Theophylline pharmacokinetics in patients with acute renal failure requiring hemodialysis are not well-established.
    • Understanding drug clearance and protein binding is essential for safe and effective therapeutic drug monitoring.

    Observation:

    • A 64-year-old male smoker with acute renal failure was studied for theophylline pharmacokinetics.
    • Hemodialysis clearance of theophylline was measured at 39.4 ml/kg/hr, indicating significant removal.
    • Total body clearance was 52.4 ml/kg/hr, similar to non-renal failure smokers.

    Findings:

    • Theophylline is significantly hemodialyzed, with a clearance comparable to previous reports.
    • Renal failure did not substantially alter the total body clearance of theophylline in this patient.
    • Plasma protein binding decreased from 40% pre-dialysis to 33% post-dialysis.

    Implications:

    • Acute renal failure and hemodialysis may alter theophylline's plasma protein binding.
    • Therapeutic drug monitoring of theophylline is recommended in renal failure patients undergoing hemodialysis.
    • Reduced protein binding may necessitate a lower therapeutic range for theophylline in this population.
