    Background:

    • Orbital pseudotumor diagnosis is challenging preoperatively.
    • The link between sinus disease and orbital pseudotumor remains controversial.

    Observation:

    • Two patients presented with unilateral proptosis, diplopia, palpable orbital masses, ocular discomfort, and sinus issues.
    • Echography revealed low-reflective masses in the orbit and adjacent sinuses.
    • Imaging suggested bony orbital wall erosion, leading to a presumptive diagnosis of sinus malignancy with orbital extension.

    Findings:

    • Histopathology confirmed chronic inflammatory changes consistent with pseudotumor in both sinus and nasal tissues.
    • Orbitotomy revealed vascular connections between orbital and sinus lesions and irregular bone pitting, not lytic erosion.
    • Simultaneous orbital and sinus pseudotumors represent a distinct clinicopathologic entity.

    Implications:

    • This entity requires consideration in the differential diagnosis of orbital masses.
    • Accurate preoperative diagnosis can be improved by recognizing this combined presentation.
    • Understanding the relationship between sinus and orbital inflammation is key for effective management.
