    Simultaneous orbital and sinus pseudotumors, a distinct clinicopathologic entity, present unique diagnostic challenges. Awareness of this condition is crucial for accurate diagnosis and management of orbital disease.

    Observation:

    • Two patients presented with unilateral proptosis, diplopia, palpable orbital masses, ocular discomfort, and sinus issues.
    • Echography revealed low-reflective masses in the orbit and adjacent sinuses.
    • Imaging suggested bony orbital wall erosion, leading to a presumptive diagnosis of sinus malignancy with orbital extension.

    Findings:

    • Histopathology confirmed chronic inflammatory changes consistent with pseudotumor in both sinus and nasal tissues.
    • Orbitotomy revealed vascular connections between orbital and sinus lesions and irregular bone pitting, not lytic erosion.
