Disopyramide can cause fatal cardiovascular collapse in patients with severe heart failure and kidney problems. This drug is contraindicated in these patients due to risks of prolonged QRS and QTc intervals, leading to death.

    Area of Science:

    • Cardiology
    • Clinical Pharmacology

    Background:

    • Disopyramide is an antiarrhythmic medication.
    • Severe left ventricular failure and renal insufficiency are common comorbidities.

    Observation:

    • Five patients with severe heart failure and renal insufficiency experienced cardiovascular collapse and death after disopyramide initiation.
    • ECG changes, including QRS and QTc prolongation, preceded collapse.

    Findings:

    • Disopyramide administration led to progressive lengthening of ventricular depolarization and repolarization.
    • All patients exhibited sinus bradycardia, atrioventricular block, or both.
    • High terminal disopyramide blood concentrations were noted in two patients.

    Implications:

    Related Experiment Videos

  • Disopyramide is contraindicated in patients with severe heart failure and renal insufficiency.
  • Progressive widening of QRS or QT intervals may indicate impending severe myocardial dysfunction and risk of collapse.
