    Background:

    • Open-angle glaucoma (OAG) management often requires lowering intraocular pressure (IOP).
    • Standard filtering surgery carries risks and may not be suitable for all patients.

    Observation:

    • Thirty-five phakic eyes with uncontrolled OAG received 360-degree argon laser trabecular surgery.
    • Intraocular pressure was monitored, with untreated eyes serving as controls when feasible.

    Findings:

    • A statistically significant mean IOP reduction of 10.0 mm Hg was observed at four months (P < 0.001).
    • Sustained IOP control (<20 mm Hg) was achieved in 97% of eyes over 18 months.
    • Improved outflow facility was noted for six months post-treatment, indicating the procedure's mechanism.
