    Background:

    • The vitelline duct, a remnant of embryonic development, can be associated with various gastrointestinal anomalies.

    • Meckel's diverticulum is a common congenital anomaly, but its association with specific ileal malformations causing chronic blood loss is less understood.

    Observation:

    • Three adult patients presented with chronic anemia attributed to unusual ileal malformations.
    • The malformations were located at the site of the vitelline duct remnant.
    • Each case exhibited localized mega-ileum and a non-specific ulcer responsible for chronic blood loss.

    Findings:

    • Two patients had malformations associated with Meckel's diverticulum.
    • One patient had a similar malformation linked to a vascular abnormality, likely of vitelline artery origin.
    • A previously undescribed syndrome of adult segmental ileal dilatation with non-specific ulcer and chronic anemia was identified.

    Implications:

    • This finding expands the spectrum of gastrointestinal anomalies associated with vitelline duct remnants.
    • It highlights a potential cause of obscure chronic blood loss and anemia in adults.
    • Recognition of this syndrome is crucial for accurate diagnosis and appropriate management of affected individuals.
