Ticrynafen-induced acute renal failure

D Powers, N D Vaziri, K Muhalwas

    Clinical Toxicology
    |April 1, 1981
    PubMed
    Summary

Ticrynafen can cause acute kidney injury by increasing uric acid levels, leading to kidney damage. Prompt treatment with fluids and sodium bicarbonate helped patients fully recover renal function.

    Area of Science:

    • Nephrology
    • Pharmacology

    Background:

    • Ticrynafen, a uricosuric agent, was prescribed for hyperuricemia.
    • Patients had pre-existing mild hyperuricemia and were on hydrochlorothiazide therapy.

    Observation:

    • Two patients developed acute renal failure, flank pain, oliguria, and azotemia after starting ticrynafen.
    • Clinical presentation suggested acute uric acid nephropathy.
    • Intravenous urography revealed radiolucent material in the renal pelvis of one patient.

    Findings:

    • Ticrynafen induced significant hyperuricosuria.
    • This led to intrarenal and extrarenal uric acid deposition.
    • Pre-existing hyperuricemia and volume depletion were contributing factors.

    Implications:

    • Ticrynafen use requires careful monitoring in patients with risk factors for uric acid nephropathy.
    • Understanding ticrynafen's effect on uric acid metabolism is crucial for preventing renal complications.
    • Management involves hydration, alkalinization, and potentially uric acid-lowering agents.
