    Background:

    • Gastric emptying is commonly measured using radionuclide-labeled foods.
    • It is often assumed that a single labeled food represents the emptying rate of the entire meal.
    • This assumption has not been rigorously tested for meals containing diverse food types and particle sizes.

    Purpose of the Study:

    • To investigate whether different foods and particle sizes within a single meal empty from the human stomach at different rates.
    • To determine if a single radionuclide-labeled component accurately reflects the emptying of the entire meal.

    Main Methods:

    • Eight volunteers consumed a meal containing beef stew, water, and chicken liver (half as 0.25-mm particles, half as 10-mm chunks), labeled with technetium-99m (99mTc) and indium-113m (113mIn).
    • Another eight volunteers ingested noodles labeled with iodine-123 (123I) and liver labeled with 113mIn, along with water.
    • Gastric emptying was monitored for 3 hours using gamma camera imaging, with corrections for radioactive decay and spectral interference.

    Main Results:

    • 0.25-mm liver particles emptied significantly faster (t 1/2 = 70 min) than 10-mm liver chunks (t 1/2 = 117 min) in 7 out of 8 subjects (p < 0.05).
    • Noodles emptied significantly faster (t 1/2 = 52 min) than liver (t 1/2 = 82 min) in 6 out of 8 subjects (p < 0.02).
    • The emptying rate of 10-mm liver differed between the two meal compositions (p < 0.05), suggesting meal composition influences gastric motility.

    Conclusions:

    • Different foods and particle sizes within a meal exhibit distinct gastric emptying rates.
    • Using a single radionuclide-labeled food may not accurately represent the emptying kinetics of all meal components.
    • Meal composition can influence the gastric emptying of specific food components.
