    This study reports successful surgical removal of large gallstones causing small intestine obstruction in two patients with biliary ileus. The cases highlight effective enterotomy techniques for resolving this rare gastrointestinal condition.

    Background:

    • Biliary ileus is a rare condition where gallstones obstruct the gastrointestinal tract.
    • Gallstone impaction in the small intestine can lead to significant morbidity.

    Observation:

    • Two cases of biliary ileus with large calculi obstructing the small intestine are presented.
    • Calculi were located at different points within the small intestine: 1 meter from Treitz fascia and 1.5 meters from the ileocecal valve.

    Findings:

    • Successful enterotomy and calculus removal were achieved in both reported cases.
    • The study describes the aetiopathogenesis, clinical presentation, and treatment strategies for biliary ileus.

    Implications:

    • Demonstrates the efficacy of surgical intervention for gallstone-induced small bowel obstruction.
