Leiomyomatosis in pelvic lymph nodes

Y K Hsu, N B Rosenshein, T H Parmley

    Obstetrics and Gynecology
    |June 1, 1981

    Summary

During pregnancy, pelvic lymph nodes can show a decidual reaction, mimicking smooth muscle tumors. This can lead to misdiagnosis of cancer or benign metastasizing leiomyoma.

    Area of Science:

    • Gynecologic Pathology
    • Reproductive Medicine
    • Surgical Pathology

    Background:

    • Normal pregnancy involves a decidual reaction in pelvic tissues.
    • Pelvic lymph nodes can undergo significant changes during gestation.
    • Understanding these changes is crucial for accurate pathological diagnosis.

    Observation:

    • A decidual reaction, typically seen in the uterus, can manifest in pelvic lymph nodes.
    • This decidual reaction may be replaced by a metaplastic process resembling leiomyomatosis peritonealis disseminata.
    • The presence of smooth muscle-like tissue in lymph nodes is an unusual finding.

    Findings:

    • The metaplastic changes in pelvic lymph nodes can mimic smooth muscle neoplasms.
    • This can lead to a differential diagnosis including metastatic leiomyosarcoma.
    • Benign metastasizing leiomyoma is another potential misdiagnosis.

    Implications:

    • Accurate pathological interpretation is essential to avoid misdiagnosis of malignancy.
    • Awareness of these benign reactive changes can prevent unnecessary aggressive treatment.
    • This highlights the importance of clinicopathological correlation in gynecologic pathology.
