    Summary

    Many insomnia patients have underlying sleep disorders, not just psychological issues. Polysomnography (sleep studies) can differentiate these disorders, guiding better insomnia treatment strategies.

    How representative are insomnia clinical trials?

    Background:

    • Insomnia is a common complaint, often treated without a clear underlying diagnosis.
    • The relationship between subjective sleep complaints and objective sleep disturbances is not fully understood.

    Purpose of the Study:

    • To categorize patients with insomnia based on comprehensive evaluations.
    • To determine the prevalence of objective sleep disorders in patients complaining of insomnia.
    • To explore the utility of polysomnography in differentiating insomnia subtypes.

    Main Methods:

    • 84 patients with insomnia complaints underwent medical, psychiatric, and polysomnographic evaluations.
    • Objective sleep parameters were compared to 20 normal control subjects.
    • The Minnesota Multiphasic Personality Inventory (MMPI) was used to assess psychological distress.

    Main Results:

    • Only 50% of insomnia patients had objectively defined sleep difficulties.
    • Polysomnography revealed a sleep disorder in all but 16 patients.
    • Insomnia diagnostic categories were distinguishable by polysomnography.
    • Psychological distress was evident in only 3 of 10 diagnostic categories.

    Conclusions:

    • A significant proportion of insomnia complaints are linked to underlying, objectively identifiable sleep disorders.
    • Polysomnography is crucial for accurate insomnia diagnosis and subtyping.
    • Insomnia treatment should consider objective sleep findings, not solely psychological factors.
