    Summary

Conservative management is effective for dilated non-obstructed renal pelvis. This study followed 28 patients, finding 25 remained well with satisfactory renal function after 1-5 years, confirming a non-surgical approach.

    Area of Science:

    • Pediatric Urology
    • Diagnostic Imaging
    • Nephrology

    Background:

    • Renal pelvic dilatation (RPD) can be caused by obstruction or be a non-obstructive finding.
    • Historically, RPD often led to surgical intervention like pyeloplasty.
    • Accurate diagnosis is crucial to avoid unnecessary procedures.

    Purpose of the Study:

    • To evaluate the long-term outcomes of patients with RPD where obstruction was excluded.
    • To determine the efficacy of conservative management for non-obstructed RPD.
    • To assess the role of excretion urography and diuresis renography in diagnosis and management.

    Main Methods:

    • Retrospective analysis of 28 patients with RPD.
    • Exclusion of obstruction using excretion urography and diuresis renography.
    • Clinical follow-up for 1 to 5 years, including repeat renography.

    Main Results:

    • Obstruction was excluded in all 28 patients.
    • 25 out of 28 patients remained well with satisfactory renal function and excretion.
    • Only one patient required pyeloplasty; others had minor or no complications.

    Conclusions:

    • Conservative management is the appropriate approach for dilated non-obstructed renal pelvis.
    • Objective functional assessments (urography, renography) enable conservative treatment.
    • Avoidance of unnecessary pyeloplasty is possible with accurate diagnostic methods.
