    Summary

    A previously undescribed chronic nonparoxysmal junctional tachycardia in an adult was identified. Electrophysiologic studies confirmed a benign His bundle rhythm, not requiring treatment.

    Background:

    • Acquired nonparoxysmal junctional tachycardia is a rare cardiac arrhythmia.
    • Chronic forms of this rhythm are particularly uncommon and poorly described.
    • Understanding the electrophysiologic characteristics is crucial for diagnosis and management.

    Observation:

    • A case of chronic (3-year duration) acquired nonparoxysmal junctional tachycardia in an adult is presented.
    • Ambulatory monitoring showed junctional rates between 75-110 bpm.
    • Electrophysiologic studies revealed normal sinus node function and intact AV/VA conduction.

    Findings:

    • The tachycardia originated from the proximal His bundle, indicated by rate increase with autonomic stimulation.
    • A narrow QRS complex and normal H-V interval were observed.
    • The rhythm was determined to be benign based on electrocardiographic and electrophysiologic findings.

    Implications:

    • This case expands the understanding of junctional tachycardia variations.
    • The findings suggest that some chronic junctional tachycardias may be asymptomatic and benign.
    • It highlights the importance of comprehensive electrophysiologic evaluation for unexplained tachycardias.
