    Background:

    Observation:

    • A young woman presented with a large, asymptomatic unilateral adnexal mass six weeks postpartum.
    • Abdominal and pelvic examinations at parturition revealed no adnexal abnormalities.

    Findings:

    • Laparotomy identified a 20 cm right ovarian cyst.
    • Microscopic examination confirmed the cyst was of corpus luteum origin.
    • The cyst's size, rapid development, and postpartum presentation were highly unusual.

    Implications:

    • This case underscores the importance of vigilant postpartum surveillance for adnexal masses.
    • Corpus luteum cysts can exceptionally reach significant sizes and persist postpartum.
    • Further investigation into the etiology of rapid, large postpartum adnexal mass development is warranted.
