Carcinoma in choledochal cyst

Background:

Choledochal cysts are congenital bile duct dilatations.
Association with adenocarcinoma presents diagnostic challenges.

Observation:

The classical triad of jaundice, abdominal mass, and pain lacks specificity for diagnosing choledochal cysts with malignancy.
Operative cholangiography is crucial for identifying filling defects suggestive of malignancy within the cyst.

Findings:

Malignancy in choledochal cysts was diagnosed intraoperatively and confirmed via histopathological examination in both documented cases.
Combined internal drainage procedures and 5-fluorouracil (5-FU) chemotherapy resulted in asymptomatic recovery for both patients.

Implications:

Highlights the limitations of clinical presentation in diagnosing choledochal cyst adenocarcinoma.
Emphasizes the diagnostic value of operative cholangiograms and the efficacy of combined surgical and chemotherapeutic management.
