    Background:

    • This report details a 52-year-old man experiencing unilateral INO after a head injury from a car accident.
    • The patient exhibited gradual improvement in INO symptoms over a six-month follow-up period.

    Findings:

    • Head trauma is an infrequent cause of internuclear ophthalmoplegia.
    • The literature review identified only ten prior cases of INO secondary to head injury.
    • Of these, seven were direct trauma results, and three occurred post-subdural hematoma formation.

    Implications:

    • This case contributes to the understanding of rare causes of internuclear ophthalmoplegia.
    • It underscores the need to consider head trauma in the differential diagnosis of INO, particularly in unilateral presentations.
