    Summary

    T3 contamination in levothyroxine (L-T4) preparations significantly elevates serum T3 levels after acute L-T4 administration in hypothyroid patients. This T3 rise is primarily due to T3 contaminants, not extrathyroidal conversion, and can reach euthyroid levels.

    Background:

    • Levothyroxine (L-T4) is the standard treatment for hypothyroidism.
    • Extrathyroidal conversion of T4 to T3 normally sustains circulating T3 levels.
