    Background:

    • Intraocular lens (IOL) implantation can lead to complications such as retropseudophakic membranes and vitreous adhesion.
    • Persistent cystoid macular edema (CME) and dislocated IOLs pose challenges in pseudophakic eyes.
    • Vitreous in the anterior segment can complicate secondary IOL implantation.

    Observation:

    • Closed vitrectomy was utilized in 33 eyes with existing intraocular lenses.
    • The procedure addressed specific complications including membrane removal, vitreous strand management, and IOL repositioning.
    • Vitrectomy was also performed to clear the anterior segment before secondary IOL placement.

    Findings:

    • Successful removal of retropseudophakic membranes was achieved.
    • Vitreous strands adherent to the IOL, incisions, and iris were effectively managed.
    • Dislocated IOLs were relocated, and pseudophakic pupillary block was relieved.

    Implications:

    • Closed vitrectomy is a viable and successful surgical option for managing diverse complications following IOL implantation.
    • The technique offers solutions for persistent CME and anterior segment vitreous issues.
    • This approach can improve visual function and patient outcomes in complex pseudophakic eyes.
