Tibiotalar tilt--a new slant

H Griffiths, J Wandtke

    Summary

Tibiotalar tilt (TTT) is linked to more than 20 conditions beyond the classically known four. Its cause is likely related to stress and blood supply issues in the distal tibial epiphysis.

    Area of Science:

    • Orthopedics
    • Pediatric Rheumatology
    • Genetics

    Background:

    • Tibiotalar tilt (TTT) is traditionally associated with Fairbanks disease, hemophilia, sickle-cell anemia, and juvenile rheumatoid arthritis.
    • Recent findings indicate TTT is present in a broader spectrum of conditions.

    Purpose of the Study:

    • To expand the known associations of tibiotalar tilt.
    • To explore the pathogenesis of TTT.
    • To differentiate TTT from pseudotibiotalar tilt.

    Main Methods:

    • Literature review of TTT cases.
    • Analysis of radiographic findings.
    • Clinical case study review.

    Main Results:

    • TTT was identified in over 20 additional conditions, including various dysplasias, fibrous dysplasia, metabolic diseases, and post-traumatic cases.
    • The most probable pathogenesis involves stress and compromised blood supply to the distal tibial epiphysis.
    • Key differentiating features between TTT and pseudotibiotalar tilt were identified.

    Conclusions:

    • The etiology of TTT is more diverse than previously recognized.
    • Distal tibial epiphyseal stress and vascularity are likely key factors in TTT development.
    • Accurate differentiation from pseudotibiotalar tilt is crucial for diagnosis and management.
