    Summary

    Fasting significantly improved rheumatoid arthritis (RA) symptoms and increased the urine cyclic AMP/cyclic GMP ratio. Plasma and urine cyclic AMP levels decreased during fasting in RA patients.

    Background:

    • Rheumatoid arthritis (RA) involves complex inflammatory processes.
    • Cyclic nucleotides like cAMP and cGMP may play regulatory roles in RA.
    • Previous claims suggest fasting benefits RA patients.

    Purpose of the Study:

    • To measure plasma cAMP and urinary cGMP in untreated RA patients.
    • To investigate the effects of fasting on these cyclic nucleotides.
    • To correlate changes with clinical and laboratory measures of RA activity.

    Main Methods:

    • A cross-over study design was used.
    • 12 female RA patients participated, with control and 7-day fasting periods.
    • Plasma cAMP and urinary cGMP were measured; clinical and lab variables were assessed.

    Main Results:

    • Clinical and laboratory measures of RA inflammation significantly improved during fasting.
    • The urine cAMP/cGMP ratio increased significantly during fasting, peaking mid-fast.
