    A rare case of colitis cystica profunda, a condition causing deep mucous cysts in the colon, was found diffusely throughout the large intestine in a patient with ulcerative colitis. This extensive presentation is highly unusual and highlights diagnostic challenges.

    Background:

    • Ulcerative colitis (UC) is a chronic inflammatory bowel disease.
    • Colitis cystica profunda (CCP) is an uncommon, benign rectal condition characterized by submucosal mucous cysts.
    • CCP is often misdiagnosed as colorectal cancer.

    Observation:

    • A 20-year-old male with a 2-year history of medically refractory ulcerative colitis underwent a total proctocolectomy.
    • The surgical specimen revealed ulcerative colitis involving the descending colon and rectum.
    • Additionally, prominent, circumferential, polypoid mucosal elevations and submucosal mucous cysts, consistent with colitis cystica profunda, were observed diffusely throughout the ascending and transverse colon.

    Findings:

    • The patient presented with an extremely rare manifestation of colitis cystica profunda, with diffuse involvement extending into the proximal colon.
    • This contrasts with the typical rectal localization of CCP.
    • The pathological findings underscore the potential for CCP to mimic invasive carcinoma.

    Implications:

    • This case highlights the importance of considering CCP in the differential diagnosis of colonic polyposis, especially in patients with inflammatory bowel disease.
    • The diffuse proximal colonic involvement represents a rare variant that may pose diagnostic and therapeutic challenges.
    • Accurate pathological diagnosis is crucial to avoid misclassification as malignancy and ensure appropriate patient management.
