    Summary

    Warm water immersion for nephrotic syndrome patients led to significant weight loss and increased sodium excretion. This novel therapy warrants further investigation for diuretic-resistant cases.

    Background:

    • Nephrotic syndrome is a kidney disorder characterized by edema and proteinuria.
    • Conventional diuretic therapy is not always effective for managing fluid overload in nephrotic patients.

    Purpose of the Study:

    • To investigate the effects of neck-deep warm water immersion on fluid and electrolyte balance in nephrotic syndrome patients.
    • To explore water immersion as a potential therapeutic strategy for refractory nephrotic syndrome.

    Main Methods:

    • Five adult patients with nephrotic syndrome underwent four-hour neck-deep immersion in 1.3 m of warm water.
    • Measurements included weight loss, urine output, urine sodium excretion, and plasma/urine osmolality.
    • Aldosterone levels were monitored in a subset of patients.

    Main Results:

    • A mean weight loss of 2 kg was observed, with 1 kg from sweat and 1 kg from urine.
    • Urinary sodium excretion increased significantly (35 mmol in four hours), approximately 15 times higher than control days.
    • Urine became hypotonic to plasma within two hours; aldosterone levels showed variable changes.

    Conclusions:

    • Neck-deep warm water immersion effectively promotes diuresis and natriuresis in nephrotic syndrome patients.
    • This hydrotherapy approach shows promise as an adjunctive treatment for nephrotic syndrome unresponsive to standard diuretics.
    • Further clinical investigation is recommended for this non-pharmacological therapeutic option.
