    Summary

Most vision defects in school entrants go undetected during preschool screenings. Early detection of vision impairment requires a review of current preschool vision testing methods.

    Area of Science:

    • Ophthalmology
    • Pediatric Health
    • Public Health

    Background:

    • Vision impairment affects a significant percentage of school-aged children.
    • Early detection of vision defects is crucial for timely intervention and management.
    • Preschool examinations are intended to identify potential health issues, including vision problems, before school entry.

    Purpose of the Study:

    • To assess the effectiveness of preschool examinations in detecting vision defects among school entrants.
    • To identify the proportion of vision impairments missed during the preschool period.
    • To understand factors influencing the detection of vision problems in early childhood.

    Main Methods:

    • A cohort of 4239 school entrants was screened for distance and near vision impairment.
    • Children with vision impairment (6/12 or worse in either eye) were further investigated.
    • Data on preschool detection and age at diagnosis were analyzed.

    Main Results:

    • 3.4% of children had impaired distance vision, and 0.8% had impaired near vision.
    • Two-thirds of vision defects were not detected during preschool examinations.
    • Most diagnosed cases were identified between 3 and 5 years of age; clinic attendance was not a significant factor.

    Conclusions:

    • Current preschool vision screening methods are insufficient for early detection of many vision defects.
    • The lack of suitable testing techniques, rather than expertise, appears to be a barrier.
    • A comprehensive review of preschool vision testing strategies is recommended to improve early identification rates.
