    Summary

    Left ventricular thrombus can be detected noninvasively using echocardiography. A large, mobile thrombus causing stroke despite anticoagulation may necessitate surgical removal.

    Background:

    • Two-dimensional echocardiography enables noninvasive assessment of left ventricular thrombus.
    • Idiopathic congestive cardiomyopathy can lead to left ventricular thrombus formation.

    Observation:

    • A case of a large, pedunculated, mobile left ventricular thrombus developed in a patient with idiopathic congestive cardiomyopathy.

    Findings:

    • The patient experienced a fatal cerebral embolization.
    • The embolization occurred despite full-dose intravenous anticoagulation therapy.

    Implications:

    • Large, mobile left ventricular thrombi pose a significant risk of embolization.
    • Surgical intervention may be required for left ventricular thrombi in cases of embolization, even with adequate anticoagulation.
