    Summary

    Systemic mastocytosis symptoms, including gastrointestinal and dermatological issues, were studied in a 54-year-old woman. Cimetidine effectively managed her symptoms, suggesting its therapeutic role.

    Background:

    • Systemic mastocytosis is characterized by gastrointestinal and dermatological symptoms.
    • Patient was previously treated with cyproheptadine, diphenoxylate-atropine, and cimetidine.

    Observation:

    • The patient discontinued cimetidine (an H2-receptor antagonist) and experienced symptom recurrence.
    • Symptoms subsided upon restarting cimetidine therapy.
    • Laboratory values (histamine, gastrin, CBC, serum chemistry, urinalysis) remained normal throughout the study.

    Findings:

    • Symptom exacerbations correlated with the absence of cimetidine in serum.
    • Cimetidine treatment led to symptom resolution.

    Implications:

    • Cimetidine appears to play a role in managing the symptoms of systemic mastocytosis.
    • Further research may explore cimetidine's efficacy in a broader patient population.
