    Early psoriasis development involves polymorphonuclear leukocytes in the skin. These immune cells infiltrate the epidermis, forming pinpoint papules before typical psoriatic scaling or proliferation occurs.

    Background:

    • Psoriasis is a chronic inflammatory skin condition.
    • The earliest cellular events in psoriasis development are not fully understood.

    Purpose of the Study:

    • To investigate the initial cellular changes in uninvolved skin of patients with active psoriasis.
    • To identify the role of specific immune cells in the preclinical stages of psoriatic lesion formation.

    Main Methods:

    • Observation of demarcated skin areas in psoriasis patients and healthy volunteers.
    • Histological and histochemical analysis of early skin changes (prepinpoint papules).
    • Immunofluorescence studies to detect immunoglobulin and complement deposition.

    Main Results:

    • Erythematous, non-scaling papules (prepinpoint papules) developed in 54% of psoriasis patients.
    • Histology revealed polymorphonuclear infiltrates penetrating the epidermis in prepinpoint papules.
    • Immunoglobulins and complement were detected in a subset of these early lesions.

    Conclusions:

    • Polymorphonuclear leukocytes play a significant role in the earliest stages of psoriatic lesion development.
    • These findings suggest an early immune-mediated process preceding characteristic psoriatic epidermal changes.
