    Summary

    Thyroid hormone levels, including thyroxine (T4) and triiodothyronine (T3), are altered in acute renal failure (ARF). Abnormal peripheral T4 metabolism is the likely cause of these changes in thyroid function during ARF.

    Background:

    • Acute renal failure (ARF) can significantly impact various physiological systems.
    • Thyroid hormone levels and metabolism are known to be affected by critical illness.

    Purpose of the Study:

    • To investigate thyroid function in patients with acute renal failure (ARF) across different disease phases.
    • To determine the relationship between ARF and alterations in thyroid hormone concentrations and metabolism.

    Main Methods:

    • Evaluation of thyroid function in 36 patients with ARF.
    • Measurement of serum thyroxine (T4), triiodothyronine (T3), reverse triiodothyronine (rT3), and thyreotropin levels during oliguric/anuric, polyuric, and postpolyuric phases.
    • Assessment of the sephadex-T3-binding index (T3I).

    Main Results:

    • Serum T4 and T3 concentrations were significantly decreased during the oliguric/anuric phase of ARF.
    • Serum rT3 levels were elevated in the oliguric/anuric phase and normalized in the polyuric phase.
    • Serum thyreotropin levels were elevated throughout all phases of ARF.
    • T3I was significantly increased in both oliguric/anuric and polyuric phases.

    Conclusions:

    • Altered peripheral metabolism of thyroxine (T4) appears to be the primary driver of abnormal thyroid hormone concentrations in ARF.
    • Thyroid function is significantly disturbed during ARF, with distinct patterns observed across different phases of renal recovery.
