South African Medical Journal = Suid-Afrikaanse Tydskrif Vir Geneeskunde
    |October 10, 1981

    Summary

Sarcoidosis can affect the heart, causing dangerous arrhythmias and heart failure. Treatment with amiodarone and prednisone effectively controlled these cardiac symptoms in one patient.

    Area of Science:

    • Cardiology
    • Immunology
    • Pathology

    Background:

    • Cardiac involvement in sarcoidosis is a significant clinical concern.
    • Sarcoidosis can lead to diverse and severe cardiac manifestations.

    Observation:

    • A 63-year-old male patient with a history of sarcoidosis presented with recurrent ventricular tachyarrhythmias.
    • The patient also experienced intermittent heart block and congestive cardiac failure.

    Findings:

    • Transvenous endomyocardial biopsy confirmed myocardial infiltration by sarcoidosis.
    • Amiodarone and prednisone therapy successfully abolished the patient's ventricular tachyarrhythmias.

    Implications:

    • This case highlights the importance of considering cardiac sarcoidosis in patients with unexplained arrhythmias.
    • Pharmacological management, including amiodarone and prednisone, can be effective in controlling cardiac manifestations of sarcoidosis.
    • Endomyocardial biopsy remains a crucial diagnostic tool for confirming cardiac sarcoidosis.
